Abstract:
The recent introduction of intracanal cryotherapy has been shown to reduce postoperative pain following nonsurgical root canal therapy. The current protocol recommends an additional irrigation with 20 mL of chilled (2.5°C) sterile saline immediately prior to obturation. However, such a protocol adds time to a procedure that already demands extensive irrigation. The purpose of this in vitro study was to determine if the antibacterial properties of sodium hypochlorite (NaOCl), ethylenediaminetetraacetic acid (EDTA), and chlorhexidine (CHX) are affected by reducing their temperatures to 2.5°C. Streptococcus mutans biofilms were grown on dentin discs and treated with NaOCl, EDTA, or CHX at three different temperatures: 2.5°C, 23°C, and 37°C. The viability of organisms was quantified using a tetrazolium dye colorimetric (MTT) assay. The antibacterial capability of the irrigants at each temperature was then compared using the Kruskal-Wallis and post-hoc Mann-Whitney U-test and Tukey's test. Untreated and sterile discs were used as positive and negative controls, respectively. Scanning electron microscopy (SEM) was conducted to evaluate for qualitative visual differences. The 2.5°C irrigants demonstrated significantly greater bacterial reduction (p < 0.05), with no significant difference between the specimens treated at 23°C and 37°C. Evaluators found no differences in visual appearance via SEM. This study suggests that using chilled (2.5°C) irrigants during endodontic treatment does not compromise the antibacterial properties of the irrigants. This treatment strategy may save valuable chair time. Clinical trials are needed to confirm these findings.
