Abstract: To determine factors associated with success and failure in vital pulp therapy (VPT) in immature permanent teeth, a retrospective chart analysis was conducted at Rutgers School of Dental Medicine Postgraduate Endodontics and Pedodontics clinics between July 2015 and July 2023. Three VPT techniques were evaluated clinically and radiographically: direct pulp capping, partial pulpotomy, and full pulpotomy. Preoperative factors assessed include the extent of pulp exposure, stage of root development, and pulpal and periapical diagnoses. Intraoperative factors assessed include the type of pulp capping material used. Postoperative factors, including the type of permanent restoration and the time elapsed between the completion of the procedure and the final restoration, were assessed. VPTs were classified as successful or failures based on clinical and radiographic criteria. Factors were assessed with Cox regression (α = .05). A total of 453 patient charts were reviewed; 130 patients were lost to recall, and 96 met the inclusion criteria. Of the 96 charts included, 60 VPTs (62.5%) were considered successful at the minimum 1-year recall period. Thirty-six VPTs were considered failures, with the patient requiring RCT or extraction at the 3-month, 6-month, or 1-year recall visit. There were no significant predictors of the outcome. VPT is a promising treatment option for the conservative management of carious or traumatic pulpal involvement in immature permanent teeth.
