A permanent first molar compromised by caries may present challenges for the young patient. The immature tooth presents with large canals and open apices which complicate obturation of the root structure. While there are many options to maintain a compromised permanent first molar, the endodontist should consider extraction as a viable treatment option in certain situations. This session will help the endodontist better understand the challenges patients may face during the course of treatment. The evidence supporting the most appropriate time for extraction will be reviewed.
