Abstract: We sought to determine the effect of postoperative 0.5% bupivacaine with 1:200,000 epinephrine, a long-acting local anesthetic, on the quality of life (QOL), pain, and analgesic consumption following endodontic surgery. Patients presenting for endodontic surgery were eligible to participate and were randomly assigned to one of three groups. Immediately following surgery, Group 1 received a supplemental injection of bupivacaine, Group 2 received a simulated injection, and Group 3 did not receive an injection. Patients were given a questionnaire to complete preoperatively and postoperatively for seven days. QOL, pain, and analgesic consumption were assessed using a modified version of the Oral Health Impact Profile questionnaire. Mixed model analysis was used to compare the three study outcomes between the three groups. QOL was significantly higher, and pain levels were significantly lower in Group 1 when compared to Group 2. Patients in Group 1 were less likely to consume analgesics than patients in Group 3. Of the 87 patients analyzed, 71 underwent treatment on maxillary teeth and 16 on mandibular teeth. Patients with mandibular surgeries reported significantly more pain postoperatively than patients with maxillary surgeries across all interventions. Data collection is ongoing, and further analysis will be done to determine the differences in maxillary and mandibular teeth.Postoperative injection of bupivacaine resulted in improved QOL and less pain when compared to a mock injection and fewer analgesics consumed when compared to no injection..
