As endodontists we encounter “failing” root canal treatments on a regular basis. These cases can often be treated successfully with various methods of treatment, all sharing the same objective of saving natural teeth as much as possible. This point counterpoint presentation will discuss numerous clinical challenges of orthograde non-healing endodontic treatments and guide the clinician through accurate diagnosis, effective treatment planning, and execution of clinical procedures yielding favorable outcomes that are supported by the best available evidence. Rationale of surgical and non-surgical retreatment steps will be emphasized, incorporating recent technology and clinical tips to provide predictable and reproducible patient-centered outcomes.
