(PO12) Prognostic Factors of Endodontic Microsurgery Of Maxillary Molars With Follow-Up Over 3 Years: A CBCT Analysis

Abstract: To analyze the treatment outcomes of endodontic microsurgery (EMS) of maxillary molars in a cohort after a minimum follow-up of 3 years and to identify their related prognostic factors. A cohort of patients subjected to EMS of maxillary molars (January 2013 - December 2018), with complete clinical and pre-operative CBCT records for at least 3 years, was reviewed retrospectively (IRB #ERI21014). Cumulative survival rates were calculated by the Kaplan–Meier analysis, and their associated CBCT profile characteristics were analyzed by Pearson chi-squared test and logistic regression to identify prognostic factors that influenced the outcome (SPSS version 25.0). Seventy teeth were included (31.4% men, 68.6% women). The mean follow-up period was 57.1 months (range: 36-165 months), with a 3-year survival rate of 94.3%. Patients with diabetes had significantly lower success (p < .05). CBCT analysis revealed that the presence of furcation involvement and distance between the root apex and the sinus floor were significant factors in success (p < .05). Of the 66 teeth that survived over 3 years analyzed, radiographic success was present in 81.8%. EMS of maxillary molars resulted in a 94.3% 3-year survival rate, with the mean healing rate at 81.8% after 3-12.7 years of follow-up. CBCT analysis could provide significant prognostic factors for treatment success, and further studies are needed to establish the attributing factors for successful treatment outcomes and also the reasons for failed cases in long-term perspectives.
