(TC16) Management of Invasive Cervical Resorption Interfering with Orthodontic Tooth Movement: A Case Report

Abstract: It has been shown that Invasive Cervical Resorption (ICR) can develop ankylosis, which can potentially impede orthodontic tooth movement. Most reported cases on the negative effects of ICR on orthodontic tooth movement are associated with unerupted teeth, where surgical intervention (extraction) has been the most commonly recommended treatment. To the best of our knowledge, there have been no reports on a non-surgical treatment of an ankylosed ICR with subsequent restored orthodontic tooth movement. The current case report illustrates a patient with an erupted maxillary anterior tooth that had an ankylosed ICR lesion. Orthodontic tooth movement had failed to properly align the tooth, and instead, the patient developed an open bite as a result of orthodontic anchorage forces on adjacent teeth. The tooth was treated endodontically, with the resorption and ankylosis being treated simultaneously through an internal approach. One month after the endodontic treatment, the tooth started moving, and within two months, it was properly aligned, and the open bite was closed. Nine months after the endodontic treatment, the tooth was diagnosed with a vertical root fracture and had to be extracted. This report constitutes a unique documented case of employing a root resorption repair procedure through an internal approach to restore orthodontic tooth movement. Further investigation on this type of repair is required to properly study the long-term survival of the tooth.
