(PO132) Three-Dimensional Volumetric Analysis on the Healing and Outcome of Endodontic Microsurgery: a Cohort Study

Abstract: Studies have investigated the outcome of endodontic microsurgery (EMS) using cone beam computed tomography (CBCT). There is a need for volumetric analysis of outcomes of EMS, especially comparisons between preoperative and follow-up CBCT. The aims of this study are to assess the outcome of EMS using 3-dimensional (3D) imaging and volumetric analysis; identify prognostic factors; and investigate the impact of taking preoperative CBCT on outcomes. A total of 291 patients (367 teeth) that received EMS between 2015 and 2021 were included in this study, and 161 patients (202/367 teeth) had both a preoperative radiograph and CBCT imaging. After a minimum of 12 months, 78 patients (104/202 teeth) returned for follow-up examination, radiographs, and CBCT imaging. The preoperative and follow-up volumes of apical lesions were calculated by segmentation using 3D Slicer software (Boston, MA, USA). Follow-up CBCT scans were evaluated for healing. Prognostic factors were analyzed. Imaging analysis was performed by two calibrated postgraduate students (inter-rater reliability: .970). Currently, 30/104 teeth have been analyzed. The following results are based on the analysis thus far: the mean preoperative lesion volume is 26.17 mm³, with a mean difference of -81.79% in successful cases and +17.38% in failed cases. The average follow-up period is 33.7 months (range: 12-93 months). Preoperative CBCT (p = .249), presence of the buccal plate (p = .281), and lesion volume (p = 0.170) do not seem to impact healing after EMS.. Data analysis will be completed by the AAE24 meeting.
